LarryA systematic review of patient-reported outcomes of laparoscopic versus open cholecystectomy.
The aim of this study was to review systematically the literature reporting patient-reported outcomes after laparoscopic and open cholecystectomy. PubMed, CINAHL, EMBASE, the Cochrane library and manual searches were performed to identify randomized controlled trials (RCTs) and cohort studies of patients undergoing cholecystectomy for cholelithiasis, between January 2000 and May 2011. Quality assessment was performed using the Cochrane risk of bias tool and the Newcastle-Ottawa scale, and data extraction was undertaken independently by 2 reviewers. Main outcomes were postoperative pain and quality of life. Seventeen RCTs, 511 participants (257 laparoscopic and 264 open) and 2 cohort studies, 4,094 participants (1,902 laparoscopic and 1,192 open) were included in this study. These studies reported a significant decrease in pain at 1-3 weeks postoperatively in patients undergoing laparoscopic surgery. However, at 3 months, there was no difference between laparoscopic and open cholecystectomy. With respect to quality of life, the heterogeneity of studies precluded meta-analysis.
