To prospectively compare the effect of n-butyl ester of dehydroepiandrosterone (NDHEA) with placebo in modifying the need for rescue mechanical ventilation (MV) and 28-day mortality in severe acute respiratory distress syndrome (ARDS) patients. Ninety-two ARDS patients were prospectively randomized in a double-blind fashion to receive NDHEA or placebo. For 7 days, NDHEA was administrated once daily before rehabilitation. The primary endpoint was the need for rescue MV. The patients were randomized within 6 h of ARDS diagnosis. The secondary endpoints were 28-day mortality, lung injury score (LIS), and adverse events. Thirty-two patients were randomized to NDHEA and 30 to placebo. Baseline and clinical characteristics were similar. Mechanical ventilation was started at a median of 5 (4-6) days. Thirty-five patients (40.2%) died during the 28-day follow-up, and 11 (13%) died within 7 days of randomization. The NDHEA group received less rescue MV (87.5% versus 50%; p < 0.001). The mortality was not different between the two groups (28.7% NDHEA versus 21.5% placebo; p = 0.47).
